The GSPAA (Green Hornets) will have this form as part of our on-line registration process that is to be read and reviewed by the athlete and parents, electronically approved and will be filed as part of the registration sign-up process. There will be a mandatory check-box for acknowledging the form has been read and approved in the on-line sign up process moving forward.
